Overlapping label

The issue is caused by the style definition:

    display: block;
    font-weight: 800;
    margin-bottom: 0.5em;
    float: left;
}

Specifically, the CSS rule: float: left;

It is located in the “https://shivhareclinic.com/wp-content/themes/doctor-clinic/style.css” file of the theme active on your WordPress.

To prevent the styles in theme affect your form, you can enter the style definition below through the “Customize Form Design” attribute in the “Form Settings” tab (https://resources.developers4web.com/cff/images/documentation/form-settings-tab.png)

float:none;
}

If you want to display the labels beside the input tags in the form, you should select the “Left aligned” attribute for the “Label Placement” attribute in the same “Form Settings” tab.
