    Preface: Taking over for another developer, I did not setup the plugin.

    Previous developer used CPT to create multiple custom post types. One of them is for Locations. I want to setup a permalink structure of /<state>/postname.

    I can’t get the Page Attributes to appear on the page. I have checked the box under Supports.

    What do I need to do to get these Page Attributes to appear so I can setup parent documents, etc?

    The post type needs to be set to be hierarchical if you want to be able to assign parent/child relationships.

    Both are set. These developers did something that I can’t find that is messing this up.

    When I set Hierarchical to true and Supports Page Attributes to checked, I lose the page title/permalink area completely.

    It is very confusing on this end. I just want to setup the locations with the correct url structure.

    If I recall right, if you leave the “supports” section checkboxes all empty, it’ll use the default title/editor etc. The moment you check any of them, only the ones you check are used, so you’d need to check the editor/title fields as well, effectively selecting each one you want to show up.
