Page code – will pay

Have a form that a user inputs data into. The form needs to be posted to a page that will email the results to a designated email adress, then send the user to a thank you page. The resutls need to be emailed in XML/ADF format (https://www.cobaltgroup.com/adf/ADFVersion1.pdf). Would someone want to write the form “post” page for me? Will pay.
