Page name displays as ?page_id=8

I set up the permalinks for the site (/index.php/%category%/%postname%/%page_id%/). It seems like it works for everything except the page url.
It still displays as ?page_id=8 instead of the page name (ex. our story). I am not sure what to do to fix it. I have been researching the issue and tried many things ( .htacess, looked to see if is hard coded somewhere). Talk with the hosting company, no luck. The template I used was Webby Blue.
