Page with only one service

I have a client who needs to present two scheduling options on their website, one for online consultation and the other for in-person scheduling.

The online appointment will be booked on one page and the local appointment on another page.

I need the booking form to be cleaner and more direct. There is no reason for the customer to be able to choose the type of service in this case.

So, how to display a single category on the page?
