Page/Browser/Database Cache Debug Not Displayed

Hi,

i have installed w3tc 0.9.4.1
i have installed APC on my server
enabled all settings.

enabled debug info,

but when i load my site, only this debug info is displayed, apparently different from all the other debug info i have seen for others

cant figure it out for days.
tried deleting the plugin and reinstall, doesnt help.
i do not have few mod requirements installed, but if i read correctly, those mod requirements are needed only if im using disk enhanced.

can anyone tell me what mistake i have done?
