PayPal Integration

Hi

Does anyone know of a good plugin to integrate PayPal. I have looked at a lot but they all seem a little complicated for my simple needs.

I need to have a form on my web site that customers enter the following fields before clicking the PayPal button to complete the purchase:

Name
Address
Tel no
Destination
Travel Dates.

That’s all I need, I do not need a shopping cart or anything fancy, just simple fields for the above so that I get this info when they complete through PayPal.

I have tried to contact PayPal to ask for this but they like to make things very complicated.
