PDF Form fields linking to my database

  • Can anyone assist me with this problem, I want to have a PDF form on my website for people who visit the site to fill out and to then submit or print off. But I also want to capture everything in the pdf forms into a table in my database. How do I go about creating the pdf form and adding these functionalities?
