    This can be achieved using PDF Integration add-on of Form Maker plugin. It allows sending submitted forms in PDF format. You can choose the fields you want to add to PDF file, as well as to insert additional content.

    The PDF attachment can be sent both to the admin and the user. Please have a look at its overview page and let us know in case you have questions.
