PDF forms have disappeared

I have 3 forms using this plugin, all of which were working fine last week, and generating the PDFs that I have spent time formatting to look how I wanted.

However, now, the PDF code has been replaced with the default:

<p>Votre nom : [your-name]</p>

<p>Votre email : [your-email]</p>

<p>Sujet : [your-subject] </p>

<p>Votre message : [your-message]</p>

And all of my customisations are gone. This is going to create a LOT of work for me to do it all over again. Is there anywhere that I can restore this information from?

I’ve also noticed that it doesn’t seem to be reliable in sending the PDFs consistently once set up… but I’ll deal with that after I hopefully get my customisations back.
