PDF not populating

PDF Generator is passing all form tags (i.e. [ReportQuarter]) to PDF instead of value entered by the person filling out the form. Please advise.

Here is a PDF I generated by filling out the form, submitting, and then retrieving this from the admin email it was sent to. As you can see, no form values populate the PDF.
