People having problems submitting

Hello, please check out the form on starcitymusicnotes.com contact page at this link: https://starcitymusicnotes.com/contact-form/ . It seems that the first time you fill out the form you get an error message and then you have to clear the form and resubmit. (I have had this happen from computer) it also appears that people trying to enter the form from mobile devices can not submit at all. Please help!
