Permalink Failure

I have an issue with the permalink creation for a new document being uploaded. It seems that there may be something different going on during the upload process than before.

As you can see, there are numerous files uploaded. Currently, on all but one of the files, when you click on the file and hover over the Go To Post link you can see the permalink set for the post name. The file that this is not working on is the Fox Point Amenities Rules file. It’ll show https://foxpointva.com/?post_type=mdocs-posts&p=554

If I switch my permalinks to the plain format most will be listed in the following manner:
https://foxpointva.com/?mdocs-posts=fox-point-amended-declaration&mdocs-cat=
Where the file with the issue will show as:
https://foxpointva.com/?post_type=mdocs-posts&p=554

I wasn’t sure if it was a specific file/post issue so I uploaded another test document and it is listed in the ?post_type=mdocs-posts&p=554 manner

I switch back the permalinks to the post name format and the file with ?post_type=mdocs-posts&p=554 format goes unchanged and the rest of the files are adjusted to the post name.
