Permalink Issues – Subpages Returning Homepage

When I enable permalinks, the subpages are all returning the homepage instead of their actual page. Here is the link to the website: https://www.horizon-health.org/test/

When you go to any subpage, the homepage shows up, although the url is correct: https://www.horizon-health.org/test/about-us/

If I turn off permalinks, the pages display fine.

I contacted the hosting company, and they said: The .htaccess file is now being processed and mod_rewrite is working.
