permalink structure changing

I am using wordpress ver. 2.6.2

my permalink structure keeps changing without me even doing anything to do. I am using the /%postname%/ custom structure and it keeps changing automatically. It was fine until a couple days ago. It screws everything up and I cannot access any of the posts on my site until I fix it. Please help. Thanks.
