Personal Header for the default theme

What files need to be edited to use a self-made header called personalheader.jpg for the WP default theme? What files and what do I need to replace that exists there now? Also, need to know how to keep the program from posting the name of the blog over the top of the name I have already created on the header itself. Would leaving the name of the blog blank in the admin profile page work for that?
