Personalize form design

To remove bullets you need to add styles for wp-crm form in your theme.
To remove words inside the boxes you need to remove Notes in Settings/Data tab – https://www.screencast.com/t/PRleUkAwY

I already removed the notes in Data tab but its shows the text.

Ok, then they also should be hidden in your theme styles.
