phone field missing

When plugin is enabled, the phone field completely disappears from the checkout page. When I disable plugin, and view standard checkout page, phone field is there.

Any suggestions on what might be happening or what I can do to fix this?

I sent a bug request but I’m not sure if I did it correctly or if it reached you.
