Photo uploads keep glitching

Help!

I run a covid testing center website, our online Forminator form asks people to submit a picture of their ID and the front and back of their insurance cards.

When people submit all 3 photos (through 3 different single file upload fields), only 1 photo comes through, and is referenced by name by the form submission for all 3 fields. Each photo gets renamed “image-7.jpg” no matter what the original file was named.

The file upload function seems to consistently glitch ONLY when the pictures are uploaded through an iPhone or iPad.

I pay for a wordpress screen-sharing tech support service, they went through my settings and have no idea what could be causing it. Can you guys offer any insight?
