PHP Notice errors

    If I uncheck the “Style Forms in WordPress” button in settings, I get a notice error from PHP on the ActiveDemand settings screen:

    Notice: Undefined index: activedemand_ignore_form_style in /var/www/opi.dev/wp-content/plugins/activedemand/ActiveDEMAND.php on line 201

    I also get a notice error on the page where the AD shortcode is being called:

    Notice: Undefined index: activedemand_ignore_form_style in /var/www/opi.dev/wp-content/plugins/activedemand/ActiveDEMAND.php on line 79
