PHP showing as text?

Hello, I have a website hydra2.com. Im trying to create a form on the top of the page where it says help me. When i click the save button i have it to go to hydra2.com/accountinformationinsert.php so i can use GET to save the values to my mysql. However all the code on the accountinformationsert.php is showing up as text?

I added this code in functions.php as a shortcode because im using wp bakery. Im unsure what the issue is.
