Physical Request Failure

Our form submits perfectly in Firefox, but errors out in Chrome and Safari. Instead of submitting, we get a “physical request failure” at the top of the page and “Warning: Invalid argument supplied for foreach() in /home4/q3k6o3v0/peachtreehillsplace.com/wp-content/plugins/ninja-forms/deprecated/includes/fields/calc.php on line 847” under every “List” part of the form.

Here is the Debug report for a failure from Chrome. I can send the successful report from Firefox too if that is helpful. Thanks.
