Pie register page help

Hello,
Today i have installed the plugin pie register.
But i not see th registration form.
Only what i want to do is this:

If anyone click on buy now button then i will there an fill information like this:

Email:
password:
First name:
Last name:
city:
Country: (like a menu and choose in which country you live)
Payment method:

If anyone has making a fault like (samplihotmail.com) the @ is not there and i will said like this sample: (Email not true)
A welcome message if he is an member, automatic email send to the people who buy my product.
I self can make this all, but you need databse and more installation. I'm afraid I will do something on my database.

Can you help my step by step to make this all.
