piping image name to a form

Hello, My wife owns an art studio and offers guided painting classes. She has a gallery of sample class paintings that customers/groups can choose from to paint during there class. I want to be able to have a customer click on the painting they want for their class and be redirected to a form they can fill out with their details. Is there a way to pipe the painting’s name they clicked on in to a form?
