Pirate forms plugin From address changed to "username@box###.BlueHost.com"

Hello,
Since I implemented the Pirate Forms plugin, the From field on my notification emails has “username@box###.BlueHost.com” instead of the sender’s email.

Are there special instructions for the settings fields so the From field will be the senders email address?
