Pitch Premium errors after 3.6.1 upgrade

1. After upgrade to wp 3.6.1, I get this:

I changed wp debug to false in order to suppress these errors but I don’t know how to fix it. Numerous support requests have been unanswered.

2. In your project documentation video, I noticed that you choose an image from a gallery in the media browser. I don’t have that. Also when I upload images I get this error:
