Placeholders disappear after refresh in Internet Explorer 9

I’ve been wracking my brain for hours trying to figure out why my placeholders keep disappearing after I refresh the page in Internet Explorer 9. I’ve checked the page in IE8, and no issues. There are also no issues in Firefox or Google Chrome, just IE9.
Has anyone run into this issue before and been successful in fixing said issue?
If not, is there a way to get rid of the placeholders and put them above the forms?
