Placement of form not as expected

Hello. I’m editing a website for a friend and am having problems with the location of the Yikes form.

You will see that the container is somehow displaced from the form content. This seems to be causing problems with the page when it loads as the position of the text is not fixed. What I am trying to achieve is the form consistently centred. Any advice would be hugely appreciated. Thanks.
