    Try to change the site theme, it must be a CSS issue.

    I went to your link and opened it in both Firefox and Safari and I see the box in both browsers.

    The same issue is back.

    In Safari, the Please enter your email does not show up.

    In Firefox it does, but it doesn’t pass that email address to the administrator. It only lets the user know by email how they did.

    In all cases, we are trying to have the user email go to the administrator for follow up.

    I have changed the theme to test for CSS issues.
