Please Help – What is this contact plugin or feature?

    Hi everyone.

    It seems I’ve activated a feature or plugin, and now cannot identify the source of its effect on my default contact page.

    The page in question is here:
    https://reikitechnician.com/contact/

    I have inserted a picture to the page, but other than that, it contains no content whatsoever.

    The contact form that appears is in its default format. I need help identifying either the plugin or where I activated the feature so that I can understand how to use it (or customize it).

    This is a network install.

    I suspect that it’s a Jetpack feature, but have not been able to find the controls to toggle or cutomize it if that is the case.

    I’ve deactivated and reactivated a number of plugins, and it remains… then I began to test it against the default WP and BP themes, and it disappeared. It seems the contact form only appears when an Elegant Theme is applied.
