Just to confirm some details to help use longer term debug why this is happening, this is a form that was created before upgrading to version 1.9.0? It’s seemed to be a bigger issue for upgrading people over new users or at least new forms.

Which leads to a next idea. Create a second form that has the same fields, and see if that succeeds. There shouldn’t be much to worry about for “migrating over” from the first form, the data would go to the same place. Worth a try on a demo page, and we’d be happy to help test it out.

I’ve already done that. The form shown on the link I’ve provided was created just two hours ago.
