Please Try Again Infinite Loop

When in WordPress, when trying to add a new post or edit an existing post, the Upload/Insert function does not work. I merely want to upload an image to display the thumbnail properly on the site. When going to Add A New Image I am given the following: ‘Are you sure you want to do this?

Please Try Again’ in the new window that pops up. By clicking Please Try Again the add new page/edit new page reloads in the smaller pop up window.

As you can see, this is an infinite loop and I’m unaware what the problem is.

Notes:
– This is not the 2.5 or whatever it was error, when I searched that was all I found.
– No plugins were updated, modified, etc right before this occurred.
– No changes in code to WordPress were made, but my forums instead but they are in their respective folders.
– No updates occurred in WordPress right before this error.
– Yes, I’ve read the uploader sticky thread. I have attempted most of suggestions there that I thought would coincide with my problem.
