Plugin activated / Menu not working

Hi, I installed the plugin and reviewed the settings. Not working at all. I am thinking their might be an error regarding the header and or footer files based on another support thread I reviewed.

Please take a look and let me know if I am missing something! I have spent almost two weeks trying to get this silly menu to work!
