Plugin Advice

I have been at my wits end trying to find a simple plugin for 2 days. I need a plugin where I can ask questions like a survey but then the answers can be done in stars
ie: How was the attendance at the event? *******
And they rate from 1-10 or 20 in stars and then the vote is tabulated on the page so you can see the result.
I would like this to be a 20 question survey. Surely what I am asking for is not that complicated. Please help me out!
