[Plugin: Contact Form 7] checkbox code showing, but not actual checkboxes

This form works great except for the checkbox tag. I use the “Generate Tag” tool to create a checkbox but when I view the site, all I see is [checkbox products “Stamped Concrete” “Stamped Overlay” “Acid Stain” “Sprayed Overlay” “Plain Concrete”] and not the actual checkbox.

All other fields work fine. Tested on both Safari and Firefox on Mac and IE7 on PC.

I am using the newest WordPress and Plugin versions.

Any suggestions?
