Been using CF7 on many sites for quite some time and this is the only time I remember getting an error like this or having any trouble at all.

I have wp_header() and wp_footer() called in my theme, have deactivated all plugins and still no dice…is this a possible sever configuration issue?

The contact forms exist in the DB – I can see them in phpMyAdmin – as I create new forms they are added. Can’t figure out why this is happening.

I feel like an addict or something – I gotta have my freakin’ CF7 forms or I am not sure what I am going to do….

    Where is the site?

I did not post the site link as it is (admittedly) an “adult” fetish site. I did not want to get in trouble for posting link – if you tell me it’s ok I will post link to contact form page.

Site is about to go live and I had to plug in another contact form in the meantime to keep client happy.

FYI – I have done the “standard” procedure for resolving plugin conflicts – disabled all other plugins, etc. I have not switched themes – if it is theme specific it’s gotta be in the header file (maybe a javascript conflict? Error console shows nothing) as the page code is VERY simple. About as simple as one can get.
Also tried various builds of CF7 – currently back one version.

Probably you have form-field with name=”name”. Try to rename the name ;).
