[Plugin: Contact Form 7] Does not send

I’m running WP 2.8.4 on IX Web Hosting (Windows/IIS) with PHP 5.2.9. Below are the fields in the form:

[text work-phone class:pafInput]
[text cell-phone class:pafInput]
[email* email class:pafInput]
[select how-to-contact “cell phone” “home phone” “work phone” “email”]
[select asking-about “owner financing” “lease options” “selling my home” “general question”]
[textarea* message class:pafInput]
[submit “Send”]

When I submit the form, it doesn’t alert me on any of the required fields, and it never submits the email – the rotating icon just sits there.

Help!
