[Plugin: Contact Form] issues with checkboxes

  • Working on a survey design, but have ? popping up next to check-box and text of ‘option’ … not sure how to get rid of all these displays of ? in the survey when viewing it.

[checkbox* HowlonghaveyoulivedintheLacLaBicheregion exclusive "0-3 years" "4-8 years" "9+ years"]  </p>

    Displays when viewing the form:
    How long have you lived in the Lac La Biche region?
    [] & nbsp;?0-3 years [] & nbsp;?4-8 years []?& nbsp; 9+ years

    I typed in the [] to show that the checkboxes are appearing, but I cannot figure out why there is a ? displaying – it is not coded in like that, and doesn’t appear anywhere. HELP PLEASE. Page on website is currently private as the form is not launched.
