[Plugin: Contact Us Form] Not working correctly

On the 4th page of my slider at truthinseo.com, I inserted both [CONTACT-US-FORM] and ‘showForm(); ?>’. Neither of them worked. Below is the code I have for the page before it and the page I’m having issues with installing your Contact Us form:
