[Plugin: Custom-Contact-Form] Adding Extra Text to the form email

Hi, I am using the Custom-Contact-Form to allow a viewer to digitally sign a terms and conditions page. I would like the entire terms and conditions text to be emailed along with the form so the email can be printed all together. I’m not sure how I would do this using this plugin. I have placed the form shortcode after the terms text on a page so none of the page text gets emailed with the form. How can I add (or copy) this text so that it is emailed as well. Thanks for the very useful plugin!
