[Plugin: Custom Contact Forms] datepicker and styling issues in IE9

Hello CCF support,

I am running into a small issue with the CCF plugin. On IE9, I was forced to place the browser into IE8 compatibility mode to stop a datapicker error and to get the styling to look right. Main page in question is https://www.nordicblueconsulting.com/testimonials. Currently, with the compatibility mode, the styling looks proper, but the form tips are not working. It’s not a huge deal, since I was able to get the styling to look correct, but I wanted to pick your brain and see if you had any thoughts.

I am running the latest, 5.0.0.1, version.
