[Plugin: Custom Contact Forms] multisite 3.1

Wondering if this plugin actually works with the multisite 3.1, i can add forms to the main domain that the network is setup on, on any of the other sites however i can see an option to pull it into a widget but the drop down for which form to choose doesnt show any, if i use the form display code it doesnt recognise that at all in the pages or posts

Anyone else get multiple forms up for multiple domains successfully and if so, how do you do that ?
