[Plugin: Custom Field Template] Custom field template admin errors

So whenever i enable this plugin on a specific site, it spits out a bunch of errors in the backend, above all of the content.

Im confused as to where these errors are coming from as ive used this plugin before with no problems. Normally this would not be a huge problem, but in the admin section for the plugin itself it also spits out a ton of markup into each textfield eg:

Has anyone experienced this before? Does anyone have a solution?
