plugin doesn't recognize some groups / User Attributes

I’m using this plugin and having partial success on the integration:

I can log in at the AD with the test tool and the users are created at the wordpress database when I check the relevant boxes. But I can’t get the plugin to recognize the user attributes department and title, and the bulk import of all users also isn’t working: Bulk import finds just 7 users on the “domain users;id:513” groups. Another security group was created with all the users put on it, but the bulk import isn’t recognizing these groups.

I can share code / site addresses by email if this helps to diagnose the problem. I can also provide any configuration details of my setup here.
