    That’s really strange, I’ve not seen that before. Presumably you can see the form if you inspect the HTML. If yes, it sounds like a javascript problem and potentially related to the browser (I have tested various versions of IE, Chrome, Firefox). If no, a configuration problem. Can you give me a link to access the page?
