[plugin: email-to-download] several serious problems here

I am having some problems with the email to download plugin.

After inserting the short code as the installation notes says to, the original link does not show up on the page. Instead, the form iteself is embedded. After filling out the form, the download link does appear though. I believe this is supposed to give a pop-up box after clicking on the original file link as set up in download manager.
And finally, upon clicking the link, it goes to a file not found page instead of starting to download. The download manager shortcode for the same thing works fine.

Here is my download manager code that works ok by itself:
[download id=”3″ format=”4″]

Here is the code I am having the above problems with. The contact form id is 5:
[email-download download_id=”3″ contact_form_id=”5″]

I am also wondering if I can use the format code that’s in the download manager shortcode within the “email-download” shortcode?
