[Plugin: Exclusive Content Password Protect] Add a form via shortcode to the preview area?

Hello, I’d like to add a contact form via shortcode into the preview area, above the “enter password to unlock” div.

I assume something like this:

echo do_shortcode(‘[contact form shortcode name]’);

could be dropped in somewhere? But I’ve not had luck implementing this. Any suggestions?
