    However, I am having difficulty with the path to captcha. In my installation for this site wordpress has its own directory (wordpress) above the root directory. When captcha is enabled, I do not see the captcha image; when I hover over the links for audio, I notice that the path is incorrect, and is missing the wordpress directory reference. I also have permalinks and html in pages plugin activated.

    Where is the best place to indicate the correct path so that the captcha folders are correctly referenced?

    Check the WP settings in Admin – Settings – General
    WordPress address (URL)
    Blog address (URL)

    This is how it knows the correct path.
    If that is not working for your unusual install you have to edit the plugin php code

    Since I try to use it for a membership form, I wish to make some changes to the default settings. Can the following be implemented without editing the plug-in code?:

    1) to replace the default “Name” by two fields “First Name” and “Last Name”.

    2) to reorder fields so “E-mail” would be the last field before the “Submit” button.

    1) well almost, all the text strings can be changed in advanced options. You can change Name to First Name, then add another field for Last Name. But the Last Name won’t be next to it.

    You are free to custom edit the PHP though.

    I installed the si contact form and I got it to work for the most part. The only problem I’m having is whenever I use a gmail.com, msn.com, hotmail.com or yahoo.com accounts I get this error: “The e-mail could not be sent. Possible reason: your host may have disabled the mail() function.”

    If I put in a different email domain, like yahoo3.com or nebraskamed.com, the form works fine.

    Do you have any idea what I’m missing?

    The site is on IIS 7.0 hosted by godaddy. The URL is https://www.janitorialsolutionsinc.com. I unchecked all the DNS options.

    Maybe your web host has banned those addresses? Ask them.

    I want to make anonymous contact form for unregistered users, that why I must disable in form NAME and EMAIL ADDREESS fields – how can I do this?
