[Plugin: Fast Secure Contact Form] Form disappear … why !?!

I’ve a problem with Fast secure contact form.
It’s very strange because until now, it work perfectly !The form doesn’t appear … If I deactivate and then activate, I can see he form.
But after some hours, it disappear !!I see like this :
