[Plugin: Fast Secure Contact Form] How can I remove "Department to Contact" field?

For some reason, This form displays “Department to Contact” field at the very first item of my form? I don’t want that to be an option.
How do I remove this or prevent this field from displaying in my form? I do not see this field in the options dashboard?https://www.ads-software.com/extend/plugins/si-contact-form/
